Where do you see patients?

We offer care in-home, in-office, and through telehealth visits.

Do you accept insurance?

We accept most insurance plans in Hawaii, including Medicare and Medicaid, except Kaiser.

How quickly can I be seen?

Appointments are typically scheduled within 24–72 hours.

Can you order labs and imaging?

Yes, for established clients, we provide in-home lab draws and coordinate imaging and diagnostic services.

Do you offer in-home visits?

Yes, we specialize in bringing care directly to your home when there is a medical or behavioral necessity.

Do you manage chronic conditions?

Yes, we provide ongoing care for conditions like diabetes, hypertension, and more.

How do you do telehealth?

You will receive a secure link through Doximity to join your telehealth visit.

Does it cost anything for an in-home visit?

No, we work directly with your insurance, so there is no additional cost for in-home visits.

How do I submit a referral as a case manager or caregiver?

Send a demographic sheet and most recent visit note (if available) to patientsupport@integrated-care.org, and our team will contact the member and coordinate scheduling within 24–72 hours.
